Your story matters.

We understand how exhausting it can be to repeatedly explain your circumstances to different people and services.

At Bloomwell, we want to listen properly, understand your experiences and work alongside you and those involved in your support. You’re more than your circumstances, and your story deserves to be heard.

Tell your story once

Two people sitting across a wooden table from each other, each holding a cup of coffee.

Why it matters

Because people shouldn’t have to tell their story twice. This isn’t about any one service getting things wrong. It is what tends to happen when support is split across many organisations, each with its own forms and processes.

What many people experience before they reach supported accommodation

  • Repeating their story to multiple professionals
  • Multiple assessments
  • Housing instability
  • Crisis-driven interventions
  • Fragmented support
  • Poor communication between services

The Bloomwell difference

  • One lead support worker A consistent, trusted relationship.
  • One recovery plan Focused on your goals and what matters most to you.
  • One secure support record Kept safely in one place, so you don’t have to repeat yourself.
  • One consistent team People who know you and work together.
  • One coordinated approach Health, housing and social care working in partnership.
  • One clear pathway towards independence Building skills, confidence and supporting long-term wellbeing.

Every professional involved builds on the same understanding. That reduces repetition, builds trust and lets people focus on recovery rather than retelling their experiences.

How it works, from admission onwards

Five connected commitments shape how we work with each person from the day they join us.

It means you only have to share your story with us once, and we do the rest. We take a joined-up approach, working with you and relevant partners, so your information, needs and goals are understood from the start.

People first. Always.

  1. One assessment

We take time to understand your whole story, needs and goals.

  2. One support team

A consistent team who know you, work with you and stay by your side.

  3. One support plan

A personalised plan that brings together housing, wellbeing and practical support.

  4. One secure record

We keep what you tell us safely in one place, so you don’t have to keep repeating it. It’s only ever shared when that’s appropriate and lawful.

  5. One coordinated approach

We work in partnership with local services to give you the right support, at the right time.

So you can focus on what really matters: your future.

A clearer path

For many people, contact with services starts at a point of crisis. Our aim is to turn that into a steady path towards independence.

  1. Step 1: Crisis

Often where people’s contact with services begins.

  2. Step 2: Assessment

One careful conversation about your whole story.

  3. Step 3: Bloomwell

A safe, supportive place to begin again.

  4. Step 4: Recovery plan

    Personal, holistic and goals-focused.

  5. Step 5: Independence

Confidence, choice and a brighter future.

Everyone’s journey is different. This shows the general direction of travel, not a fixed timetable.

Your information is safe with us

Telling your story once doesn’t mean your information is passed around. Your voice should always matter in decisions about your life.

We keep one secure support record, so that the people supporting you at Bloomwell don’t need to ask you the same questions again.

Information is never shared automatically between organisations. We only share it with other services when it’s appropriate and lawful to do so. Usually that means with your agreement; occasionally the law allows or requires us to share it, for example to protect someone from serious harm.

You can ask what information we hold about you. Read our privacy notice for more about how we handle personal information.

Start with one conversation

Whether you’re looking for support or asking for someone else, we’ll listen first.
